I was curious to see how this worked (and what was broken), so I did
some checking on my machine. Running OS X 10.3.5, all the listed file
extensions worked fine before installing the patch. I made a directory
with .qcp, .wav, .mid, .mp3, and .doc files in it--also .MP3 (to see if
case was an issue).

The choose file dialog brought up from the '+' button in the Ringers
pane looked perfect without this patch--the .doc file was grayed out and
couldn't be selected, while all the other types were selectable.

With the patch, the .doc file will presumably also be selectable.

Interesting.

Attached is a screenshot that shows how this works on Windows and Linux
(and every other operating system I have ever used :-)

When you select the file type thingy then only files matching the
particular extensions are shown, as well as all directories. However
the user can always type anything they want in the "File name" field.
For example you could type in the name of a file that doesn't meet
the currently selected pattern. You can also type in your own
pattern. For example if you typed in "*.barf" as the filename and
hit Open/Ok then all files with an extension .barf will be shown.

case then it is really stupid IMNSHO. The other alternative is that
MacOS does provide a way of selecting the files and people just don't
know to do it.
I know there is such a way, but it's been a long time since I did that
so have no details in my memory.

It may well not be available in Java, though it is with native code.
I'll try to remember to look into it.
